Wellness/Disclosure Form

Dear Patients,

We hope this letter finds you and your family in good health. While many things have changed during the events of the past several months, one thing has remained the same: our office’s commitment to your health and safety.

Infection control has always been a top priority in our practice. We follow recommendations and guidelines made by the American Dental Association (ADA), the Centers for Disease Control and Prevention (CDC), and the Occupational Safety and Health Administration (OSHA). This allows us to make sure that our infection control procedures are up-to-date and enables us to provide the safest environment for our patients and our team.

During this time, we have implemented additional safety precautions to help protect our patients and staff.

Prior to your appointment, you will be asked to complete our COVID-19 wellness/disclosure form. This questionnaire must be completed before or when you arrive for your appointment. We will postpone treatment for any patient who has experienced or been in contact with someone who has experienced the following symptoms within 14 days of the scheduled appointment date: fever, cough, loss of taste and/or smell, flu-like symptoms, difficulty breathing or shortness of breath, and fatigue.

Please be aware of the following protocols on the day of your appointment:

  • Upon arrival at our office, call us before entering the office to let us know you have arrived. We will call or text you once the office is ready for you to enter.
  • Every person entering the office is expected to wear a face covering.
  • Every person entering the office will have their temperature checked with a contactless thermometer.
  • We are practicing proper social distancing when it comes to scheduling appointments and treating patients in our office. For this reason, we ask that patients enter the office unaccompanied, if possible. For those patients who need a guardian to accompany them, we ask that this be limited to a single family member.

Although you might not notice these changes, we have made the several additions to our equipment in the office, including the use of:

  • EOS Extraoral Suction Systems in every operatory during all patient treatment to reduce the amount of airborne particles
  • GermGuardian Air Purifying Towers with HEPA Filters and UV-C in every operatory

Thank you for being part of our family dental practice,

Calcagno Cosmetic and Family Dentistry
